An article in today's New York Times, "60 Years and 1,000 Tales Since 14 Were Ejected" tells of the time on July 19, 1946 when 14 White Sox players and coaches were ejected from a game against the Red Sox.

Jones, apparently unsure of the primary source, eventually ejected all but Manager Ted Lyons, [Mule] Haas, a trainer and a batboy on the bench. By several accounts, the most derisive term tossed toward Jones was meathead. Mod edit: Clipped for fair use purposes.
The article is interesting but pretty dry, and takes the angle of getting to the bottom of who it was that kept razzing the ump. It includes quotes from Sox outfielder Ralph Hodgin, 90, and a photo of third baseman Dario Lodigiani, also 90. Lodigiani contends that the culprit was third-base coach Mule Haas who had a "gift for making whoopee-cushion noises with his mouth.":D:

I knew I'd find it if I looked hard enough... There was a baseball card made of the incident. The card was issued in 1960, but they have a picture of the bench after the ejections....
