Dedication Book

I have some questions about the "Dedicated To..." book with all of our dedications in it.

1. IIRC there were some dedications that were in teal, are those included in the book, and if so, are they in teal?
2. Is the format seen on the sample on the WSI main page how it will be in the book? (i.e. Dedicated to etc...

2. I am not sure, but I would hazard a guess it is close.
