Baseball question re: last night

The Royals tied the score at 4 in the top of the 8th when a pinch runner (last name Johnson, I think) scored from FIRST on a single to right. I was there, but I was watching the runner, not the ball, and I'm not sure what happened, but the ball ended up rolling around the infield while the runner scored and the batter advanced to second (or third?). Does anyone know what happened? Did Maggs miss the cut off man? Did the cut off man drop or somehow misplay the throw?

Maggs threw it to Konerko, who let the ball bounce off the tip of his glove because he had his eye on the runner. The ball caromed off his glove all the way to the pitcher's mound, Paulie tried to throw it to the catcher, but he was too late.

It was, as they say, "not good."
