I found this on the Baseball America page. It's a chart of players who lied about their age. There are only three of our players on the list (all minor leaguers) but NINE such Flubs on the list.

Players that "aged" 5 years or more...

ANA Julio Reyes from 11-21-84 to 11-21-79
DET Franklin Perez from 5-11-80 to 5-11-75
FLA Francisco Ferrand from 5-20-80 to 5-20-73
OAK Carlos Rosario from 2-22-80 to 2-22-75
PIT Eddy Ramos from 9-28-78 to 9-28-73
SEA Aquilino Lopez from 7-30-80 to 4-21-75
SEA Gustavo Martinez from 11-9-80 to 11-9-75
STL Franklyn Tejada from 4-7-80 to 4-7-75
TB Wilmy Caceres from 10-2-78 to 10-2-73
TB Bartolome Fortunato from 7-27-80 to 8-24-74

Off the top of my head the only legit major league talents in this bunch are Lopez, a relief pitcher taken in the Rule 5 draft, and Caceres, a utility man.
